Social Support Programs

Social Support Group offers a number of programs that encourage and promote social connections, independence and well-being of older people and people living with a disability or chronic illness, whilst providing respite and support to carers. We welcome people from diverse backgrounds with provisions for individual and cultural needs.

Social Support offers

  • Weekly centre-based sessions that provide activities including gentle exercise, games to help maintain health, flexibility, mobility and self-esteem with a supportive environment that maximises the person’s wellbeing
  • Monthly community-based groups that offer trips to local and surrounding areas to help develop and maintain social networks
  • Pop up programs based on client interest that have previously included Market Makers, Movie Programs, Card and Craft Making, Bowling and Recorder Lessons
  • Special Interest groups that run weekly include Morton Sewing group, Morton card making and scrapbooking group, Morton Art group, Melton Library group and Morton and Melton Shopping program.

Volunteer

Volunteers play an integral role in the delivery of services for the aged, carers and people with a disability. There are currently opportunities available for our Social Support Program Volunteers.

Our busy Social Support Programs needs additional volunteers to assist with activities and outings.

If you would like to volunteer and:

  • are willing to support and help older people and people with disabilities engage with activities, pop up programs and outings
  • are available one or two days a week
  • are happy to make a cuppa and have a chat

Service Access

These programs are offered as part of the Federal Governments Commonwealth Home Support Program (CHSP) and the State Governments Home and Community Care Program for Younger People (HACC-PYP). They aim to support the independence, functional capacity and social participation of eligible people at home and in the community. Client contribution fees apply.

Assessment is the gateway to receiving government subsidised home and community supports. The assessment service will work with client and carers individual circumstances, and issue referrals to relevant support services such as the Melton City Council.

  • HACC PYP Assessment Officer – People under 65 years and not eligible for the NDIS (under 50 years if identifying as Aboriginal or Torres Strait Islander)
